Collaborative Care Solutions: Team Care Arrangements at Hawthorn East Medical Clinic (HEMAC)
A Team Care Arrangement (TCA) is a specialised healthcare program designed for patients dealing with chronic or terminal medical conditions and complex needs that require ongoing care from a multidisciplinary team. This arrangement is particularly beneficial for patients who already have a GP Management Plan in place and require coordinated care from at least three collaborating health or care providers. By participating in a TCA, patients gain access to five Medicare-subsidised allied health sessions per calendar year, helping to alleviate the financial burden associated with comprehensive care.
